About completing and signing forms
Client forms help you collect important information before, during or after appointments, with the option for your team to review and sign them when required.
Forms can be sent automatically based on your chosen settings or added manually to individual appointments and client profiles. Clients are notified when a form is added, so they can complete it in advance. Alternatively, you can fill in forms together or let clients complete them privately using client mode in-store.
Once a client submits a form, your team can review their answers and add any required signatures. Completed forms are saved to the client's records for future reference.
Add a form to an appointment
- Open an existing appointment.
- In the appointment view, click Actions (three dots) and select Add a form.
- To add an existing form, click Add next to it or click Create new form to create a new client form.
- To see what the form looks like before choosing it, click Preview to open it and click through the pages of the form.
The form appears under Records in the appointment view, ready for your client to complete.
Complete a form in-store
You can complete a form in person with your client directly from the appointment or their client profile.
-
Next to the form you want to complete, select Actions (three dots) and choose how you want to complete the form:
- Fill form: Fill in the form with your client and click Complete form to save it.
- Client mode: Hand your device to the client so they can complete the form privately, without seeing your calendar or other details. The client clicks Start form to fill it in and then Complete form to save it. Once finished, click Done to close client mode.
The form shows as Completed and is saved to the client’s profile for future reference. If the form needs a team member signature, it shows as Awaiting team review until it’s signed.
