Client forms overview
With Fresha, you can collect key client information using customizable forms. Whether you need consent, treatment history, or personal details, forms help you stay organized and deliver a seamless client experience, before or during their appointment.

Choose how forms are used
You can create and apply forms in two ways, depending on how and when you want to collect client details.
- Automatic forms
Attach forms to specific services so they are sent to clients automatically when an appointment is booked. This is ideal for collecting important information in advance.
- Manual forms
Add forms to any client appointment or profile whenever you choose. This is useful for in-store use.
Build custom forms
When creating a form, you can add customizable sections using client detail fields or your own questions. When you are ready, activate the form so it becomes available to use across your appointments or client profiles.
You can activate a form right away or keep it inactive until you're ready.
Complete and manage forms
All forms, whether completed in advance or during an appointment, are saved securely in each client’s profile. You can view forms at any stage, send reminders for incomplete ones, or complete them with the client in person. Completed forms can be printed or reviewed at any time, making it easy to keep client records accurate and up to date.
