When we get your text we will send you New Patient paperwork. We prefer for you to come into clinic for your 1st visit if at all possible. If you schedule an appointment & have to cancel or reschedule sooner than 24 hours before your appointment due to us not taking your insurance or you not scheduling appropriately you will be charged $150. To avoid this please verify your insurance is in network with us first before scheduling & make sure all New patient paperwork is completed before your appointment or come 20 minutes early.

This is for a brief appt. If you 1-2 concerns and less than 5-6 lab values to review  or are following up on a medication this would be an appropriate appointment to schedule. This is just to get the blood drawn for your labs and for us to send it in to the lab. For individual lab costs, contact the clinic. Remember that if you were told to come fasting, you should not eat for 12 hours before the draw. Make sure to drink lots of water before coming in.

This is an Annual Wellness Visit for those with Medicare that have been seen by providers at Health & Vitality in the past. It does not include a complete physical. This is a visit that is covered yearly at NO COST to you.  At an Annual Wellness Visit your health will be assessed. This is a screening visit to identify your health risks and develop a treatment plan to reduce these risks. This is not a visit to discuss current or existing health problems. A separate visit will be needed for that. Although, it is possible to have 2 separate visits on the same day. To do that you will need to schedule your annual wellness visit and another office visit.
