If you schedule an appointment & have to cancel or reschedule sooner than 24 hours before your appointment due to us not taking your insurance or you not scheduling appropriately you will be charged $150. This is an Annual Wellness Visit for those with Medicare that have been seen by providers at Health & Vitality in the past. It does not include a complete physical. This is a visit that is covered yearly at NO COST to you.  At an Annual Wellness Visit your health will be assessed. This is a screening visit to identify your health risks and develop a treatment plan to reduce these risks. This is not a visit to discuss current or existing health problems. A separate visit will be needed for that. Although, it is possible to have 2 separate visits on the same day. To do that you will need to schedule your annual wellness visit and another office visit.

This is time to go over and create your advanced healthcare directives. Advance directives are legal documents that provide instructions for medical care and only go into effect if you cannot communicate your own wishes.
