After meeting with one of our staff for an Introductory to our Practice Appointment you can decide to continue your health and wellness journey with online appointments or in office appointments. This in clinic visit lasts approximately 60 minutes with our Holistic, Functional, and Integrative Nurse Practitioner.

We focus on identifying and addressing the Underlying ROOT Cause of illness and disease, rather than just treating symptoms. We emphasizes a personalized approach, consider individual needs including differences in genetics, lifestyle, environment, and other factors that contribute to illness and disease. We use a variety of diagnostic tools with your detailed patient history (personal health story) being the most important.

In Clinic COMPREHENSIVE Visit

After meeting for your Comprehensive Appointment we continue your healing journey with follow up appointments. We like for our patients to follow up in 2-8 weeks depending upon your own individual health needs. This IN CLINIC visit lasts approximately 30 -45 minutes and will be with our Holistic, Functional, and Integrative Nurse Practitioner.

We continue to focus on identifying and addressing the Underlying ROOT Cause of illness and disease, rather than just treating symptoms. With this visit our focus is not on Functional, Integrative, or Holistic Health care needs. We address simple and specific health concerns / conditions. This visit can be a Virtual or an In Office Consultation.

This visit is a focused visit that lasts 10-15 minutes.

It's Your Health is here to help you achieve your health & wellness goals. We like to get to know you before your BIG COMPREHENSIVE appointment with our practice. This appointment is for patients who are brand new to our practice. We will introduce you to Functional, Integrative, and Holistic Medicine & how we use them together to bring health, wellness, & healing to our clients. We want to make sure our practice is the right fit for both of us. 

With this appointment we will met with you for 30 minutes. We will introduce you to our practice. We start to gather your health information so we can begin to establish a plan of care before your next appointment. 

This is NOT a COMPREHENSIVE APPOINTMENT. Your next appointment with our practice will provide you with a COMPREHENSIVE appointment which will be approximately 60 minutes in length. We typically like our patients to follow up with us in 2-8 weeks. Your follow up appointment will depend upon your own personal health needs.

If this is your first time meeting with us to discuss your health & wellness goals we ask you schedule an appointment under Patient Consultation. The New Patient Consultation is a 30 minute appointment where we introduce ourself to you and make sure we are the right fit for your health & wellness goals.

This appointment block is for patients who have already met with us whether in person, online, or through telehealth. This is your COMPREHENSIVE appointment. We gather additional information from you to start a personal plan for health, wellness, & healing. Please plan to spend 60 to 90 minutes with us during this appointment. This appointment will cost $300.00. 

Each individuals health, wellness, & healing journey is unique. Healing the body is a journey and follow up is essential. With this in mind we want you to anticipate follow up with us in 2-8 weeks.

Online Health Evaluation - Comprehensive Appointment

It's Your Health is here for you every step of the way. This appointment should only be scheduled if you have previously met with us for your COMPREHENSIVE Health Visit. During your last appointment we discussed when we would like for you to follow up. Please do not schedule an appointment under this tab unless we have specifically discussed a follow up plan of care and follow up appointment with you. 

We are each unique individuals. Our health & wellness needs should be individualized and unique. Your follow up appointment will vary depending upon your individual health needs.
