D4 Medical forms are available to print online, alternatively most of our doctors carry spare D4 Medical forms. 

Please bring your:
1. Photo ID, 
2. Medication list (if applicable)
3. Glasses and Eye prescription if worn for driving. 

FEE INCLUDES EYE TEST assessment.
Please note If you do not bring your eye prescription with you then, you may well need to see an optician to complete the vision assessment separately at your cost.

HGV PCV D4 C1 D1 DVLA DRIVER MEDICAL

Available only for individuals that are deemed fit to drive in line with DVLA guidance. The onus is on the individual to provide medical records and investigation results.

Please note Licencing Authorities or councils set their own regulations and rules. For some Taxi Medical assessments you may need to provide a copy of your GP Medical Records or Medical Summary. A urine sample may also be needed. Please check with us if you are not sure.

You will need to bring Photo ID and a Urine Sample with you. If you take any medication or glasses please bring them too.

Our Airside Medical includes a driver medical assessment to Group 2 standards (D4 Form), Forced Whisper test, Urine test and a colour vision test. Please check with your employer whether our Airside medical is sufficient for your needs. For additional health or work related assessments please contact us prior to booking (additional fees may apply). PLEASE BRING A URINE SAMPLE WITH YOU.

Airside Medical Driver Permit

Your fit to work certificate for plant and forklifts is usually issued on the same day. Certificates can be also be issued and emailed from our head office. Please don't forget your ID, Medication list and glasses if applicable.
