This is a cortisone shot as typically used for an inflammatory  process such as poison ivy, allergies and general aches and pains.  DOES NOT INCLUDE THE BASIC OFFICE VISIT.

This is for drivers sent via CERTEDRIVE ONLY.  not for regular DOT driver physicals. These are not paid by the driver but reimbursed by CERTEDRIVE. 

CERTEDRIVE DOT CLIENTS ONLY

This is for a DOT Driver Medical Examination.  For new drivers and for recertification.  Price includes urinalysis (except for CERTEDRIVE Drug Screens). 

DOT Driver Medical Exam

1 liter of IV fluids placed intravenously for correction of dehydration. 

IV Fluids For Rehydration

Cortisone and local anesthesia placed into the affected joint. Does not include the $65 Office Visit charge. 

Joint Injection 

Drainage or excision of cyst.  Sterile procedure. Pre-numbed.

CYST REMOVAL

Opening and removing contents of abscess and packing the wound.  Includes 3 subsequent treatments for re-packing the wound.  Includes numbing and a post procedural prescription for antibiotics. 

I&D Abcess Complex (requires packing)

Complete or partial removal of toenail for ingrowing nail.   Includes numbing and prescription for post procedure antibiotics.

General counseling, medication, monitoring and management.  Prescription but cost of medication is not included.  Costs vary by prescription.  Some insurance may cover costs.  Will work to try to get your insurance to cover it.  

Weight Loss Management Intake Visit

HRT management visit.  To recheck labs to make sure they are within a therapeutic range and make any necessary changes to the compounded medications.  These can be done every 60-90 days.

Initial intake visit and set up for labs and with compounding pharmacy for medications.  

HRT INTAKE VISIT

Bio-Identical Hormone Replacement (HRT)

Includes Breast exam, PAP test and routine female exam.  Also includes counseling and education. If a prescription is needed, the medication cost is not included.  Lab specimen will be processed through QUEST Diagnostics or the lab or your choice.  Additional fees may be included if not through QUEST.   If you have healthcare insurance it will be processed through Baxter Regional Medical Center laboratory and your healthcare insurance will cover the cost of the laboratory testing. 

FEMALE EXAM

This is for a general physical exam, medication review,  review of body systems and to fill out any necessary forms for school, work, military/coast guard and insurance, etc.

Special Medical Exam

Tests for hepatitis. A, B, C and more if indicated.   This is a serum test and requires a blood draw.  Samples will be sent to QUEST Diagnostics or to Baxter Regional Medical Center for testing and results.  If insured, your insurance should cover the cost of testing.  Self-pay patients will pay for testing at the time of exam.
