Many thanks for the review,  the van has gone in for a valet this week. A combination of muddy boots and bad weather significantly contributed to the aesthetic appearance. Apologies for the unit being 5 minutes late arrived 1115am. Appointment 1110am.

D4 Medical forms are available to print online, alternatively most of our doctors carry spare D4 Medical forms. Please bring Photo ID, Medication (if applicable) and glasses if worn for driving. FEE INCLUDES EYE TEST EXAMINATION
