Your medical chart and consents will be emailed to you and must be completed prior to your appointment or you will be rescheduled. Baseline labs required before consult for all weight loss medications. If you have recent preventative care labs from your primary care (less than 6 months old) send them to me at heather@vernillowellness.com or fax 888-561-5898 so I can review them. 
If you do not have labs please contact me for a lab script and choose an appointment slot for at least SEVEN days after you go for labs.
If you don't have insurance coverage for labs I have an affordable self pay panel.

Wellness Consultation(CPT 99213)

$45 per area treated. The Skin Classic Direct high frequency technology is an affordable non-laser treatment for minor skin irregularities such as telangiectasia, spider naevi, cherry angiomas, keratosis,fibromas, skin tags, hyperpigmentation, milia, sebaceous hyperplasia and acne pimples.  Skin Classic treatments are quick, and barely touch the surface.
